OUHSC Course Request Form (OUHSC instructors only)

HSC faculty members are invited to request a Blackboard course for any academic semester using the form below. Due to the way that Bb is integrated with our course student information system, faculty must currently request a new course - or a copied course - using the form below each semester. If you would like to request a Bb site not associated with a catalog course, please utilize the Request a Bb Community form.

Complete the following information and Submit. You will receive an email notifying you when your course is ready for you to access.
